Police forced their way into a Georgette Crescent house in Endeavour Hills at 3.30pm after the 73-year-old woman activated a personal medical alarm device she was wearing around her neck.
Officers found the woman on the floor barely conscious with a large amount of blood loss.
Casey CIU detective Senior Constable Justin Stanton said the offender had stalked his victim through the house.
"It appears the assault happened in multiple rooms," he said.
"Luckily she was wearing a MEPACS duress alarm and she activated it.
"When uniform arrived they had to force their way in and they found her on the floor, barely conscious with a lot of blood loss and they called an ambulance."
Detective Snr-Constable Stanton said no neighbours were home at the time of the attack but police were reviewing CCTV footage from several properties to help identify the attacker.
They are also trying to contact tradesmen who were working in the area this week.
The woman was taken to hospital with life-threatening injuries and was undergoing emergency surgery last night.
The victim told investigators she did not know the identity of her attacker but he was Caucasian, in his 40s and wearing a florescent vest.
Neighbour Lilly Drazic, 55, was in her garden only several houses away, but said she had not heard the attack.
“My husband was always telling me to be careful but I didn't care, now I will be closing my door for sure.
"There are people out there who are sick and wrong in the head."
Police have appealed for witnesses.

The government has deliberately lax laws regarding serious crimes of this calibre.
Since criminals know they can literally get away with murder, instances like the ones mentioned in this article will continue to increase.
The government's law makers and politicians have little regard for the safety of the masses.
One is considered more of a 'criminal' if one has not paid a speeding fine or a taxation bill.
As long as revenue is collected from the plebs is all that matters.
The story would be quite different if a member of the law maker's family was injured in a home invasion.
